Payment Terms

Net 30 days to established accounts. Please call customer service to set up an account at 602-252-5891. All other orders must be prepaid with Visa or Mastercard.

All prices are as listed. To receive a quote for a bulk order, please contact Arizona Therapy Source via phone or email. Prices are subject to change and shipping may vary depending on bulk logistics. Arizona Therapy Source reserves the right to substitute products of equal or superior quality to those shown in our catalog and/or website at any time without notice unless customer specifically requests otherwise. Arizona Therapy Source reserves the right to withhold shipment of pending orders for overdue accounts. Overdue invoices shall bear interest at 1-1/2%. Customer shall be responsible for any cost of collection.

Sales Tax & Exemptions

Out of State, Non-Profit Hospital and Resale purchases shall be the only sales receiving any tax exemption benefits. All Local and for private use Customers will be charged the Arizona standard tax amounts.

All Sales are Final. 30 day refunds are available for non-working items under warrantee/ and or damage caused by shipping. Please see shipping policies for details.
